New Patient Information
Prior to your initial appointment, you may (at your convenience) complete the New Patient Information below after reviewing the Outpatient Services Contract contained therein. You may also review the HIPPA release form below. Should you wish, Dr. Turnbull can consult other professionals on your treatment team with the release form below completed. If you wish, you may complete this paperwork in the 15 minutes prior to your first appointment. Regardless of whether you complete these forms prior to your appointment, you must provide your insurance card & driver’s license/identification at your first appointment.
